| Confirm that the data are correct, click "Confirm" button to proceed. |
 |
| You can order multiple NSO document; Birth Certificate, Marriage Certificate, Death Certificate and CENOMAR. Once all is done, click the "Submit" button. |

| Make sure to save this page by pressing CTRL + P to print or just screenshot the whole page. |
